For your servant who is sick

O Lord our God, king of ages, who by your word heal every sickness: look upon your servant who is ill. Forgive their sins, free them from every disease, and restore them to health and strength, so that they may give thanks to you in your holy Church. For you are the source of healing, and to you we give glory, to the Father, the Son and the Holy Spirit, now and ever and unto ages of ages. Amen.
